Hospitals use heavy-duty disinfectants (like sodium hypochlorite (chlorine), phenols, and quaternary ammonium compounds) for floor cleaning and sanitisation. Hospital STP also deals with wastewater from continuous handwashing stations, surgical scrubbing, equipment sterilisation, and the washing of contaminated bed linens, surgical gowns, and patient showers. The STP water at times contains heavy metals (mercury, silver), X-ray contrast media, high concentrations of antibiotics, chemotherapy drugs, and endocrine disruptors.
These toxicants have a strong antimicrobial effect, killing healthy MLVSS. The BOD-to-COD ratio is often very low (<0.3), making organic breakdown difficult. All of this puts Hospital STPs under extreme stress.

System becomes stable. No fluctuations in MLSS despite shock loads. pH fluctuations do not affect the system. The aeration tank looks visibly transformed.
MLSS levels now stay consistent. The plant runs smoother with no upsets.
Water quality improves sharply.
COD levels drop rapidly, showing strong treatment performance. The STP is now very stable.
Sludge becomes dense and settles faster. There are no floating scum issues in the settling tank. SVI improves.
The outlet water becomes clear, odour-free, and suitable for non-potable reuse applications. Many hospitals reuse this water for flushing, gardening, cooling towers, and housekeeping, reducing freshwater consumption by 40% to 60%.
